All students attending public schools must provide proof of immunity to specific communicable disease according to Washington State Law (RCW 28A.210.080). Documentation of immunization status must be provided to the student's school on or before the first day of attendance.

Exceptions to receiving vaccinations include healthcare provider verification of disease, immunity by blood test (titer), or exemption. Current laws state that most exemptions require the signature of a licensed health care professional.

Immunization forms are available below, at your child's school or through your child's healthcare provider. If you need assistance in obtaining the necessary vaccinations or documentation for your child, please contact the school nurse through contacting their school's main office.
